We are busy preparing the details for this summer. Out first order of business is to arrange for your travel to and from Hawai’i. In order to ticket you we require the following information as soon as possible.

 

 

(1)  Name EXACTLY as it appears on either your Drivers License or passport, whichever form of ID you will be using at the airport.

(2)  Date of Birth (month, day, year)

(3)  Gender

 

 

(4)  Release of all Claims

(5)  Voluntary Medical Disclosure  

(6)  Air Hold Harmless

 

Travel to CSULB: As for travel to Cal State Long Beach – We request that you arrive on (or before) June 2, 2013.  if you arrive before this date you will need to find your own accommodations. Please let us know your travel arrangements so that we can know when to expect you.
